As a working contractor, I’ve experienced learning from mistakes a few times. One is decking off scaffolding. There was a bad plank and a worker stepped on it and cracked his ribs on the scaffold post. I learned that if the plank is questionable, throw it out.

The second is rain the day before. A worker slipped and broke his ankle during a simple task using a cutter bender. Make sure there is sound, firm footing.

Third, we were doing a grout pour and the grout pump started to slip belts. We had a mechanic work on the pump the day before making adjustments, but he didn’t tighten things down. So when this happened, I needed to tighten things down and leaned over, and accidentally hit the switch to start pumping. Luckily, I was clear; it could have been a disaster. Always turn off machinery before working on it.
